Update 8/30:

I just saw the doctor for my follow up and he was very sure that this is caused by water retention caused by the filler. He said this is common right near the glans because the skin is so thin there.

He injected me with a bit of the dissolving solution right into that area and said the issue should be fixed in a few days.

We scheduled a follow up appointment in a week.

He was pretty adamant that this his nothing to do with lymph nodes or anything like that. I hope he is correct…

Update 20 minutes later:
Jesus. It now looks WAY more swollen after the dissolver was injected. I really hope this is to be expected:

Update 40 minutes later:
Got the swelling to do down by lightly massaging a bit. Also got in touch with the doctor who told me that this is a normal reaction to to the prick into the thin skin.

It seems @Dickwhitman79 has a similar issue with it getting more irritated from masturbation. Does this help at all with figuring out between lymph node vs filled migration?

@Rejuvall maybe the doctor has additional thoughts on this?

Is there any way for a doctor to find out for sure if it is filler or lymph node drainage? Like what exam would he have to do to find out?

I will ask Dr. Carney for more details about his answer when he's back in the office Tuesday. Glad to hear and see that you're doing better. Some swelling following injection is normal and should resolve once you heal.

Curious for others input on this issue. I’m just short of 2 weeks post 21cc 30% PMMA and still have some swelling. The swelling is especially noticeable at the circ scar. I’ve started wrapping at the circ scar. Any other remedies or suggestions?

I wanted to resurface this post to see if there are any updates or if Dr. Carney or Rejuvall has any further thoughts?   I am experiencing the same issue--I have a very tight circ scar (internal) just immediately below my Circumcision ring (visible).   It creates an hourglass shape and traps fluid in the Circumcision scar.   Yesterday I had some dissolver injected and am awaiting the swelling to reduce.  

Also, very curious about Dr. Carney's technique to reduce or eliminate that hourglass narrow band.

@Northloop That “hourglass” or “donut” effect is something Dr. Carney sees most often when there is a deeper, tighter circumcision scar beneath the visible circumcision line.

The difficult part is that there is really no way for any physician (even Dr. Carney) to know beforehand exactly how deep or restrictive someone’s internal circumcision scar is going to be. Two circumcisions can look very similar externally but have very different amounts of scar tissue underneath.

When that internal scar forms a tight band, swelling and edema after girth enhancement can collect around it rather than distributing evenly. That can temporarily exaggerate the difference between the treated shaft and the circumcision area and create the “donut” or hourglass appearance you’re describing. Edema and irregular contour are recognized issues after penile filler procedures generally, although the published literature does not give us much specific data on deep circumcision scars as a cause.

Where Dr. Carney’s technique differs is that he has extensive experience operating directly on penile anatomy and does not rely on a cannula to blindly sweep filler through a predetermined tissue plane. As he works, he can actually feel changes in the tissue and identify where that circumcision scar band is located. That allows him to place and shape the filler much more precisely around the scar rather than simply pushing product through or across that tight area.

That doesn’t mean the possibility of a donut effect can be eliminated completely, especially in someone with a very deep or restrictive scar, but it allows Dr. Carney to anticipate the anatomy during the procedure and substantially reduce the likelihood and severity of that contour problem.

As for someone who already has a pronounced tight band or hourglass deformity, the appropriate correction really depends on whether what you’re seeing is residual filler, edema, scar restriction, or a combination of those things. Since you just had dissolver placed, it makes sense to allow the post-injection swelling to settle before trying to judge the final contour or determine what, if anything, needs to be addressed next.
